    Immunizations

    New Jersey State Law requires that each student has a current record of immunization on file with the school nurse. The following are acceptable forms of proof of immunization:

    1.)  Official copy of  immunization record from a licensed physician

    2.)  Official copy of an original school health record form (A-45) from another New Jersey school district 

    3.)  Official copy of  immunization record from a governmental health agency 

    4.)  Official copy of a school health record from a school district outside of New Jersey but within the United States.

    If a child enters the district from certain identified high-risk countries a valid record of a Mantoux Tuberculin Skin Test must be presented. If there is no record of a Mantoux test then the Mount Holly Township School Health Services can administer an intradermal tuberculin skin test.  By state law, the Mantoux (PPD) test will only be considered valid if administered within the prior six months.

     

    Immunization Requirements

    3 & 4 year old Preschool Program/Pre-Kindergarten

    • 4 Doses DTP (Diphtheria, Tetanus, Pertussis)
    • 3 Doses Polio (OPV)
    • 1 Dose MMR (Measles, Mumps, Rubella) Dose must be administered on or after 1stbirthday
    • 1 Dose HIB (Haemophilus B)
    • 1 Dose of Varivax/Varicella (Chicken Pox)
    • Hepatitis B (Series of 3 Injections)
    • 1 Dose Pneumococcal vaccine
    • 1 Dose Influenza Vaccine administered between September 1stand December 31st.

    Kindergarten

    • 4 Doses DTP (Diphtheria, Tetanus, Pertussis) with 1 dose given after

       the 4th birthday or any 5 doses

    • 3 Doses Polio (OPV) with 1 dose given after the 4th birthday or any 4 doses
    • 2 Dose MMR (Measles, Mumps, Rubella)
    • 1 Dose of Varivax/Varicella (Chicken Pox) Dose must be administered on or after 1stbirthday
    • Hepatitis B (Series of 3 Injections)

    Students Age 7 and Older (1st thru 12th grade)

    • 3 Doses DTP (Diphtheria, Tetanus, Pertussis)
    • 1 Dose of Tdapupon entering the 6th grade
    • 3 Doses Polio (OPV)
    • 2 Dose MMR (Measles, Mumps, Rubella) Dose must be administered on or after 1stbirthday
    • 1 Dose of Meningococcalupon entering the 6th grade
    • 1 Dose of Varivax/Varicella (Chicken Pox)
    • Hepatitis B (Series of 3 Injections)

    Combination Shots:

    Comvax-Hepatitis B & Hib (Haemophilus influenza type b)

    Pediarix- DTaP, Hepatitis B, IPV (polio)

    Prevnar is Pentavalent (pneumococcal) which is given to infants.
